Authentic Form 1098-T
A genuine Form 1098-T issued by an accredited educational institution will display these markers:
- The form header clearly states 'Form 1098-T (Rev. 20XX)' with the current tax year. The revision date matches the year the form was filed.
- Box 1 contains qualified tuition and education-related expenses. The figure aligns with actual institutional charges (tuition, fees, course materials). Amounts are never round numbers like $10,000 exactly.
- The educational institution's name, address, and federal employer identification number (EIN) appear in the top left. The EIN format is 12-3456789 with proper hyphens.
- The student's name, tax identification number (SSN or ITIN), and mailing address match what the institution has on file. No obvious spelling errors or address typos.
- The form carries the official IRS seal, barcode, and OMB control number in the top right corner. Print quality is crisp, not blurry or pixelated.
- Filing information appears in boxes at the bottom: Box 5 shows whether the student is a graduate or undergraduate. The checkboxes are marked cleanly, not hastily.
